Download OJT Form here. Updates: The OJT form has been revised per the JATC as of February 1, 2020. All apprentices must begin using the new form for all dates worked as of February 1, 2020. Any OJT’s submitted on old forms will not be accepted. All OJT’s prior to February 1, 2020 must be completely filled out on the previous OJT form. Please let the training center know if you need blank OJT forms mailed to you, otherwise please print blank copies to begin using as of February 1, 2020. The Committee requested that all work hours are audited to verify that the reported hours are accurate which includes over/under reporting work hours. If an apprentices OJT hours do not match the employer reported work hours, the apprentice may be cited and dropped from the program for fraudulently submitting hours. All OJT’s are due on the 1st of the following month and WILL BE CONSIDERED DELINQUENT AFTER 4:00PM ON THE 10th of the following month. Any apprentice who is delinquent, will be placed on the DO NOT HIRE list. As an apprentice, it is your responsibility to submit to the JATC office, an accurate record of the on-the-job training hours worked on an OJT slip every month that you are an apprentice, whether you work or not. It is your responsibility to confirm that the training center receives your OJT on time. OJT's must be filled out correctly or they will be sent back to you and will not be counted as received until a properly filled out OJT form is received by the office. All apprentices are required to complete the required work hours in each work process. Please make sure to monitor your hours by category. You may request a progress report from the training center at any time. |
